Glyceryl trinitrate

Glyceryl trinitrate (GTN)(Anginine, Nitrodur, Nitrolingual)

Class: antianginal agent                                                        
Actions: The nitrates relax and dilate veins, arteries and capillaries. Lowers systemic blood pressure. Cad causes stiffening, may not get response in coronary arteries. Blood flow through healthy vessels is increased. Main effect v BP. Relaxation of the vessels decreases afterload. Decrease workload of heart.

Act directly on smooth muscle to cause relaxation (vasodilation) and depress muscle tone

•Indications
Prevention and treatment of attacks of angina pectoris (stable and unstable)
•Pharmacokinetics
–Very rapidly absorbed
Metabolised in the liver
–Excreted in the urine



Adverse Effects
–Related to vasodilatation and decreased blood flow
–CNS: Headache, dizziness and weakness
–GI: Nausea, vomiting
–CV: Hypotension
Misc: Flushing, pallor increased perspiration


•Contraindications
–Allergy
Severe anaemia
Head trauma or cerebral haemorrhage
Pregnancy and lactation

•Caution
–Hepatic or renal disease
Hypotension, hypovolaemia, and conditions that limit cardiac output

Care
TAKE PULSE PRIOR TO ADMINISTRATION
Do not swallow
Tablets are s/l only
Light sensitive
Tingle under tongue- check expiry date
Patch – every 24 hours or as per order.
-          Use gloves(can be absorbed through fingers)
-          Write time on patch

-          Do not cut patch
