Classification: narcotic and opioid analgesics S8
Action: Unknown. Binds with opiate receptors in CNS, altering both perception of and
emotional response to pain.
Adverse/Side Effects:
CNS: sedation, somnolence, clouded sensorium, euphoria, dizziness, headache, confusion,
asthenia, nervousness, hallucinations, anxiety, depression, siezures
CV: hypotension, hypertension, arrhythmias, chest pain.
GI: nausea, vomiting, constipation, ileus, abdominal pain, dry mouth, naorexia,
diarrhoea, dyspepsia
GU: urinary retention
Respiratory: respiratory depression, hypoventilation, dyspnoea, apnoea
Skin: reaction at application site (erythema, papules, oedema), pruritus
Other: physical dependence.
Drug to drug: CNS depresants, general anaesthetics, hypnotics, MAO i's , sedatives, tricyclic antidepressants.
AVOID ALCOHOL
USE CAUTION IN COPD, hepatic or renal disease, elderly or debilitated.

Transdermal system:
Adults: 50 to 100 mcg IV q 2 to 3 minutes until desired effect.
25 to 50 mcg IV or IM (Elderly to high risk patients)
Children: 2 to 12 yrs: 2 to 3 mcg/kg IV
For Chronic Pain: 1 transdermal patch 25 mcg/hours. Wear for 72 hours.
May be adjusted to q 6 days.
Lozenge initial dose 200 mcg, repeat in 30 minutes if required
